Anterior Cruciate Ligament Rupture in Sports: Physiotherapeutic Intervention and Rehabilitation Strategies

ABSTRACT
The anterior cruciate ligament (ACL) is a knee ligament providing stability by connecting the femur to the tibia. It plays a crucial role in preventing excessive movements and unwanted rotations in the knee. ACL injuries are common, especially in sports, often requiring medical and physiotherapeutic intervention. Treatment and rehabilitation involve evidence-based multidisciplinary approaches. Current perspectives emphasize personalized rehabilitation protocols, integration of innovative technologies like virtual reality, and a focus on functional optimization for improved quality of life and safe return to sports. This study aims to highlight the relationship between surgical techniques and postoperative physiotherapy in rehabilitating a ruptured anterior cruciate ligament during sports. It conducts a qualitative bibliographic analysis using databases such as MEDLINE, SciELO, PubMed, and Google Scholar over a twelve-years period in Portuguese and English. The analysis indicates that ACL injuries can occur in up to 38% of patients, with a rise in surgical techniques preserving functional remaining fibers and grafting, along with extra-articular reinforcement using iliotibial tract tape (monoloop). Physiotherapy establishes an ideal rehabilitation plan based on the biological and mechanical understanding of the ligament. To restore knee functionality close to the normal pattern, rehabilitation aims to reduce discomfort, manage inflammation, fully recover range of motion (ROM), prevent muscle atrophy, strengthen muscles, preserve proprioception, and facilitate a return to daily and sports activities. Thus, the reconstruction and functional recovery of the ACL result from a multidisciplinary professional collaboration, with optimal outcomes achieved through an ideal post-surgical physiotherapeutic plan.
